Medical Consent for Menopausal Hormone Therapy

Your Condition

You have been diagnosed with or have reported symptoms of menopause or perimenopause—a natural biological process marking the end of menstrual cycles. This condition is characterized by hormonal changes that may cause symptoms including hot flashes, night sweats, mood changes, sleep disturbances, vaginal dryness, and other discomforts.

Hormone Replacement Therapy (HRT) using estradiol and progesterone is prescribed to alleviate these symptoms by supplementing the hormones your body no longer produces in sufficient quantities.

About Your Medication

Estradiol is a form of estrogen, the primary female sex hormone. It helps relieve menopausal symptoms by replacing declining estrogen levels.

Progesterone (or progestins) is prescribed alongside estrogen for women who have not had a hysterectomy. It protects the uterine lining from overgrowth that can occur with estrogen-only therapy.

These medications are typically available in various forms including creams, patches, pills, and gels.

Potential Benefits

The use of menopausal hormone therapy may:

  • Reduce or eliminate hot flashes and night sweats
  • Improve sleep quality
  • Reduce vaginal dryness and discomfort during intercourse
  • Improve mood and reduce irritability
  • Help prevent bone loss (osteoporosis)
  • Improve overall quality of life during the menopausal transition

Possible Side Effects

While generally well-tolerated, possible side effects may include:

  • Breast tenderness or swelling
  • Headaches
  • Nausea or stomach upset
  • Bloating or fluid retention
  • Mood changes or emotional sensitivity
  • Vaginal bleeding or spotting (especially in the first few months)
  • Skin irritation at application site (for topical formulations)
  • Weight changes
  • Changes in libido

Serious Risks

Hormone replacement therapy carries potential serious risks, including:

  • Increased risk of blood clots in legs or lungs
  • Increased risk of stroke (especially in older women or those starting HRT years after menopause)
  • Increased risk of heart disease (depending on age and time since menopause)
  • Increased risk of breast cancer (particularly with long-term use of combined estrogen-progestin therapy)
  • Increased risk of endometrial cancer (if estrogen is used without progesterone in women with a uterus)
  • Gallbladder disease
  • Dementia (in women starting HRT after age 65)

The risks and benefits vary based on your age, time since menopause, personal and family medical history, and the type and duration of hormone therapy used.

Contraindications

Do not use menopausal hormone therapy if you:

  • Have or have had breast cancer
  • Have or have had endometrial cancer or other hormone-sensitive cancers
  • Have unexplained vaginal bleeding
  • Have had blood clots in your legs or lungs
  • Have had a stroke or heart attack
  • Have liver disease
  • Have a known clotting disorder
  • Are or may be pregnant
  • Have had an allergic reaction to estrogen or progesterone

Important Considerations

  • Hormone therapy should be used at the lowest effective dose for the shortest duration necessary to achieve treatment goals
  • Regular follow-up appointments are required to monitor your response and adjust treatment
  • You should have regular breast exams and mammograms as recommended
  • You should report any unusual vaginal bleeding immediately
  • Smoking increases the risks associated with hormone therapy

Alternative Treatment Options

Alternative treatments for menopausal symptoms include:

Lifestyle Modifications:

  • Regular exercise
  • Healthy diet rich in calcium and vitamin D
  • Stress reduction techniques
  • Avoiding triggers (spicy foods, alcohol, caffeine)
  • Maintaining a cool sleep environment
  • Smoking cessation

Non-Hormonal Medications:

  • SSRIs or SNRIs (antidepressants) for hot flashes and mood symptoms
  • Gabapentin for hot flashes
  • Vaginal moisturizers and lubricants for vaginal dryness

Other Therapies:

  • Cognitive behavioral therapy (CBT) for mood and sleep issues
  • Acupuncture
  • Herbal supplements (efficacy varies; discuss with your provider)

Low-Dose Vaginal Estrogen:

  • For vaginal symptoms only, with minimal systemic absorption
